Our searchable Provider Directory includes information regarding primary care providers, specialists, ancillary providers, hospitals, urgent care centers, and pharmacies that participate with the Jai Medical Systems’ provider network. Please note, this information is subject to change.
The information provided in our searchable Provider Directory is reported and validated by the participating provider at least annually, or earlier in the case of a request by a State or Federal agency.
In addition, Jai Medical Systems will independently verify the following information at least once every three (3) years and prior to being posted online:
- Provider Specialty
- Board Certification
- Hospital Affiliation
- Hospital Accreditation
If at any time a provider informs us of a change in the information presented, we will update our online searchable provider directory within 30 calendar days of notification.
