If you are already eligible for Maryland Medicaid, please contact the State of Maryland Enrollment Broker at 1-855-642-8572 and tell them that you want to enroll with Jai Medical Systems. If you are not already enrolled in the Medicaid program and want to apply or find out if you qualify, please visit Maryland Health Connection. We can also send you an application in the mail.
