Social Needs Screening Tool
On an annual basis, and in accordance with directives from the Maryland Department of Health, Jai Medical Systems will seek feedback from our members through a health-related social needs screening. Your responses to this optional screening survey will allow us to understand you better and may help us with connecting you to resources which may be able to assist you. For assistance with any health-related social needs, please dial 211.
